First Month Costs:

1) Upon qualification and approval from Lifestar, the first month is at no charge. There is a $25.00 shipping charge for shipping within the United States. Outside the United States shipping charges may be more.

Second Month Costs:

1) The cost for the Second Month of the CD-4 Restoration Protocol is $150.00 U.S. and includes all necessary products and phone support. Shipping and applicable sales tax are not included.

Third Month Costs:

1) The cost for the Third Month of the CD-4 Restoration Protocol is $150.00 U.S. and includes all necessary products and phone support. Shipping and applicable sales tax are not included.

Fourth Month Costs:

1) The cost for the Fourth Month of the CD-4 Restoration Protocol is $150.00 U.S. and includes all necessary products and phone support. Shipping and applicable sales tax are not included

•Total costs for the first second, third and fourth months:

$450.00 not including any applicable sales tax or shipping charges.

•Monthly maintenance costs after the fourth month:
